RESUMO

BACKGROUND: Evidence-based practice (EBP) is an innovative systematic problem-solving methodology that incorporates the best research evidence into clinical practice to improve patient outcomes, job satisfaction, and reduced healthcare costs. Although there are significant advances to implement EBP into military healthcare and operational settings, many barriers and challenges still exist. Civilian healthcare organizations have examined barriers and solutions to integrate EBP into clinical practice, but limited data exists to identify barriers and solutions to integrate EBP into military healthcare settings. Advancing the implementation of EBPs within military healthcare settings has the power to transform the administrative processes of healthcare management and most importantly, the delivery of healthcare for service members and beneficiaries. The purpose of this article is to present findings from a qualitative descriptive research study which analyzed data obtained during an EBP military summit. METHODS: A qualitative descriptive research study was used to examine EBP barriers and solutions to implement EBP in military healthcare settings. Participants attended a virtual 1-day military EBP summit (n = 182). As part of the summit, participants were invited to voluntarily participate in focus groups. Focus groups were conducted to gain an understanding of EBP barriers and solutions from military and civilian nurses and medics with interest and experience conducting EBP within military healthcare settings (n = 42). Focus group discussions were transcribed and analyzed by the study team. RESULTS: The study analysis identified six themes: leadership, command culture, EBP barriers (specific to MTF/operational environments), communication, infrastructure support, and outcome measures. Sub-themes identified additional dimensions military-specific barriers and solutions within the six identified themes. CONCLUSIONS: The results of this research study identify actionable tasks and recommendations to advance EBP within the military healthcare system. EBP is currently underutilized in the military healthcare system, and supportive implementation of EBP can be accomplished through enhanced leadership engagement, changing command culture, addressing EBP barriers, infrastructure, communication planning, and integration of existing national clinical and financial outcome measures. Given the critical need to further transition of military healthcare to evidence-based data driven decisions, the knowledge gained from this study can optimize readiness and advance healthcare delivered to service members and beneficiaries within the military healthcare system.

OBJECTIVE: Aim: Of this study was to investigate the level of Military Nursing Officers'(MNOS) compassion competence and their personal level of compassion at work and their correlation with the professional quality of life. PATIENTS AND METHODS: Materials and methods This is a cross-sectional study carried out from December 2019 to May 2020 using the method of convenience sampling. The study involved 235 MNOs serving in Greek Military Hospitals. A single questionnaire containing Compassion at Work index, Compassion competence scale and ProQOL 5 was used for data collection. A total of 400 questionnaires were distributed with a response rate of 58.75%. Data analysis was performed using the statistical package SPSS 22.0. RESULTS: Results: The research showed that there was a significant positive correlation of the Compassion Satisfaction score with the com¬passion at work and compassion competence scales. So, the more compassion participants had, the more satisfaction they received from the care they put into their work. Conversely, the more compassion participants had, the less burnout they felt. Regarding secondary traumatic stress, it was found that the higher the participants' score on the dimensions of compassion in the dimension "Being non-judgmental", "Being tolerant to personal distress" and "Being empathic", the lower the secondary traumatic stress they felt. The position at hospital, the score on the compassion dimension in the dimension "Experiencing the suffering of others" were found to be independently related to the Compassion Satisfaction score. Specifically, Head Nurses had a 3.86 points lower score compared to Nurse managers. Higher values in the "Experiencing the suffering of others" dimension were related to a higher Compassion Satisfaction score. Nursing Officers who scored higher in compassion dimensions such as "Being non-judgmental", "Being tolerant to personal distress" and "Being empathic", they felt lower secondary traumatic stress. CONCLUSION: Conclusions: It is really important for Military nursing Officers to be compassionate in order to get more satisfaction from caring about their work and feel less burnout.

(AU)

The main mission of the Military Pharmacy is the maintenance of the operational force deployed in missions. To this end, after the study of the Request of the Spanish Armed Forces (FFAA), it was decided to modernize the traditional loperamide tablets to the pharmaceutical form of orodispersible tablets. This change, as well as the technological manufacturing process, has been possible thanks to advances in manufacturing and control equipment and modern excipients. Military pharmacists upgrade to a combatant-friendly dosage form in all deployment situations, including initial ones where access to water is engaged. The manufacture by direct compression and the use of superdisintegrants allows savings in production time and cost and, thanks to its immediate release from the pharmaceutical form, increases the efficacy and rapidity of the drug's effects. For these reasons, constant updating to the state of the science allows for the total availability of the operational force in tactical deployments without temporary casualties of specialists that are difficult to replace and their cohesion in modern combat groups. Introduction: Aeromedical evacuation missions to transport highly infectious critical COVID-19 patients involve multiple factors, therefore planning is essential in their preparation. The management of specialist nurses is cross-cutting, as they participate in the planning, organisation and implementation of aero-evacuation protocols, delivery of care and operational safety procedures throughout the operation, with an integrated aeromedical crew working as a team alongside the air crew. Objective: To study the management of nurses specialised in the aero-evacuation of highly infectious critical patients during the COVID-19 pandemic. Design: Through a systematic search of the biomedical literature, this study was conducted according to the standards of the Cochrane Handbook for Systematic Reviews of Interventions and PRISMA. The literature search included articles published from 2019 to August 2020 and meta-search engines, yielding a total of 142 articles, and a triangulation of information was performed. A total of 11 articles were eventually included in the review, and the validity of each article was determined according to the guidelines of the Joanna Briggs Institute level of evidence. Results and discussion: The systematic review identified 11 studies that use different methodological considerations for conducting an aeromedical evacuation and covering key aspects for a successful operation. Conclusion: The management of specialist nurses has a direct impact on the aeromedical evacuation of the critical patient. It covers planning, organisation, and operational safety, applied in the instruction, training, execution and strict monitoring of the work of the aeromedical crew as a solid team. This contributes to the success of the aero-evacuation mission and the infectious patient's high likelihood of survival during the COVID-19 pandemic.(AU)

(AU)

Background: Assistance to the wounded in the combat field creates the need to integrate medicine, military tactics and nursing in a reasoned manner, the latter consisting of the administration of prehospital care, surgical procedures and stabilization of the wounded. Objective: to determine the main functions of the nursing profession in the Spanish Army and Aeronautics between 1855 and 1931. Methods: A bibliographic review of the main scientific databases and search engines, as well as specialized libraries in the history of nursing, has been carried out. The inclusion criteria were: documents that analyzed the functions of military practitioners, nurses; as well as their rank and rank. Results: Starting in 1855 with the organization of the Facultative Scale, the figure of military practitioner ceased to be a figure destined to be a future doctor to incorporate their own functions and responsibilities. The Moroccan War implied that military logistics specialized, creating aircraft for military tactics with the opening of the Cuatro Vientos aerodrome (Madrid) in 1911 and carrying out the first Spanish air evacuation mission in the same Rif War in 1922. It would not be until 1988 when the woman was able to join the Army as a professional soldier. Conclusion: The contributions of the Spanish army and aeronautics to the nursing profession lie mainly in three large fields: the extra-hospital field, the field of general and vascular surgery, and the field of traumatology. RESUMO

BACKGROUND: Although the conflict in Vietnam (usually referred to as the Vietnam War) ended almost 50 years ago, few research-based publications of nurses' experiences in Vietnam exist. PURPOSE: The purpose of this study was to expand what is known about the experiences of US military nurses who served in Vietnam. METHODS: This secondary analysis used qualitative description to examine interview data from 15 nurses who served in-country (within Vietnam) and in-theater supporting Vietnam (e.g., Guam, the Philippines) between 1965 and 1972. FINDINGS: We found that nurses' experiences varied based on time deployed and place deployed (land, sea, or air; in-country or in-theater). The influence of time and place on US military nurses' experiences in Vietnam are illustrated through findings pertaining to danger, daily life, and work. The most prominent differences were between nurses assigned in-country and those assigned in-theater. DISCUSSION: The findings illustrate ways research of more recent and future conflicts might be strengthened.

In this research we tried to analyze the influence of the family-work interrelationship with perceived stress and the general mental health of staff involved in emergencies. In addition, it has been explored whether a specific psychological variable such as perceived self-efficacy exerted some kind of mediating effect between the family-work axis relationships with outcome variables, stress and general mental health. To this end, a series of questionnaires were applied to a sample of 460 participants from the Emergency Military Unit, whose data were subsequently analyzed through linear regression techniques, and mediation study with PROCESS.Results indicate that there is a significant direct relationship between family-work interrelationship, with respect to stress and general mental health perceived by emergency staff. No evidence has been found that perceived the self-efficacy variable exerts any kind of mediating effect between family-work interrelation and outcome variables. In conclusion, further research should be carried out on psychological variables that can mitigate the stress generated by emergency personnel, from their family environment. It has been shown that this has a significant relationship between the two concepts and that it can have consequences on the general mental health of the participants.(AU)

Introduction: Exsanguinating hemorrhage is the first preventable cause of death for combatants in armed conflicts, thus making the possibility of transfusing blood components early an absolutely essential benefit during health care for casualties generated in military operations. Throughout the numerous conflicts that have occurred during the past century, there have been important changes in the hemotherapy treatment of casualties along with an evolution of the health doctrine regarding this issue. In some massive transfusion protocols, the diagnostic technique of thromboelastometry (TE) has been used. TE is a test that shows the viscoelastic properties of blood from clot formation to fibrinolysis, evaluates platelet function and correlates quickly with the physiological process of hemostasis. The main objective of this study is the evaluation in vitro of the hemostatic capacity of the various cold, frozen and fresh platelet concentrates through standardized coagulation tests and thromboelastometry, clarifying whether the contribution to the clot is significantly improved with cold platelet pools (preserved at 4ºC), compared to fresh and frozen platelets. It is also intended to determine what advantages would be the incorporation of cold platelets in transfusion medicine performed in the military environment. Material and methods: An experimental study was designed to compare cold (refrigerated), frozen and fresh (conventional) platelets in vitro, analyzing their performance and effectiveness through systematic blood analysis, routine laboratory coagulation mechanisms (Prothrombin Time, Prothrombin Activity, Cephalin and Fibrinogen) and Rotational Thromboelastometry (ROTEM).A sample of 20 healthy patients was recruited, after informing them in writing and obtaining the mandatory informed consent, they donated 6 tubes with 10 ml citrate. of blood per patient...(AU)

In the mid-twentieth century, cardiac surgery emerged thanks to John Gibbon & IBM Company heart-lung machine incorporation in Philadelphia, USA, allowing open heart surgery to be carry out, which was soon possible in most developing countries. However, a few years before, some surgeons tried to cure their patients with ingenious closed-heart surgical techniques. During World War II, several military surgeons landed a decisive role in the development of this surgery by incorporating some surgical techniques that saved the life of numerous soldiers on the battlefront, among them Dwight E. Harken and C. Walton Lillehei, worldwide considered «fathers of cardiac surgery». Several important achievements emerged later, from these pioneers, regardless of their military activity, such as the intensive care units, the first mitral valvuloplasties and mechanical artificial heart valves, the implantable cardiac pacemakers, or the use of moderate hypothermia in open heart surgery. The important contributions of these admirable surgeons constituted the building blocks on which modern cardiac surgery was built.(AU)

RESUMO

ABSTRACT: The study examined differences between military students enrolled in an accelerated bachelor of science in nursing (BSN) program with and without medical experience and/or a previous bachelor's degree on graduation grade point average (GPA) and NCLEX®-RN first-time pass rates. Significant differences were found between groups with/without military medical experience on NCLEX-RN, but not graduation GPA. Significant differences were found between groups with/without prior degree on GPA, but not on NCLEX-RN. Having both medical experience and a prior degree did not make a difference on GPA or NCLEX-RN.
